Steps:

  • Provide attached Certification of Medical Necessity and Prescription Form, filled out by your PCP and/or Ortho.

  • Contact Kempf Surgical Appliances Inc to set up an Appt.

  • Provide Kempf with your paperwork upon arrival.

  • Kempf will evaluate your area of concern and provide alternatives.

  • Kempf will work with your medical insurance to cover the cost of the solution.  Note there is no Co-pay per Kempf.
