Form, filled out by your PCP and/or Ortho

Kempf with your paperwork upon arrival
4.
Kempf will evaluate your area of concern and
provide alternatives
5.
Kempf will work with your medical insurance
to cover the cost of the solution. Note there is no Co-pay
per Kempf.
